Good day,
I am 53 years old and an estrogen dominance was diagnosed after a hormone saliva test with a ratio of estradiol/progesterone 1:57. I have been taking 3% progesterone cream for 3 weeks, but it has actually worsened symptoms such as loss of appetite, palpitations, and increased gas/bloating.
How long can this worsening last?

Unfortunately, it is not possible to provide precise answers to this, as these side effects may occur, but always vary in strength and duration on an individual basis. If your symptoms do not improve within 10-14 days, this therapy may no longer be tolerable for you.
What can you do to improve tolerability?
While I can advise you to address bloating with SAB drops or Lefax chewable tablets (both available over the counter) and improve appetite loss with an appetizer, I cannot provide advice from a distance regarding heart palpitations. If your pulse rate is below 100 beats per minute, you can wait for now.
If your heart rate is consistently over 100 beats per minute, you should discuss this with your doctor. If the cream is indeed causing the issue, it is not suitable. But there may be other causes that should be investigated locally.
