Diastolic blood pressure value too low and systolic too high

I have the following problem: My grandmother (90) was put on medication after a mild heart attack (NSTEMI). She takes two blood pressure medications in the morning (Enahexal and Amlodipine) and Clopidogrel, and Enahexal again in the evening. The heart attack is now a year ago and her blood pressure has been well controlled. However, for several weeks now, the lower value has been very low (between 50 and 60). When it is so low, she experiences discomfort and dizziness according to her reports. I measured her blood pressure for monitoring and she had a value of 165/53. She measures her blood pressure daily and skips the evening medication with Enahexal (5 mg) if the diastolic value is below 60, in order not to further lower it with the medication, which seems logical to me as a layperson. Therefore, my question: Does medication, such as Enahexal, affect both values or only lower the upper value? In that case, she could take the tablet regardless of the lower value, which would be beneficial because otherwise the problems of high systolic blood pressure may arise. Since she is very reluctant to go to the doctor, I hope you can advise me on whether such a visit is necessary given the low diastolic value or if there is even a possibility to lower only the systolic value.
