According to the laboratory results of a stool examination, I have a M2-PK of 4.97 U/g with suspected gastrointestinal neoplasia. Is the value of 4.97 concerning? Of course, I have an endoscopy scheduled.

This test is controversial in the medical field. Even with elevated values, it is not a sure sign of colorectal cancer. Since you are already planning an endoscopy, you don't need to worry about this value even more. Depending on the test kit, a cutoff value of 4 is considered positive, but the significance of this only slightly elevated value is not very high.
The gold standard for diagnosis is endoscopy. Was the fecal occult blood test positive?
