multiresistant Proteus mirabilis

I have a urinary tract infection with a multidrug-resistant Proteus mirabilis. I briefly took ciprofloxacin because the doctor mistakenly assumed it was E. coli, after the lab results I was switched to Moxifloxacin 400. I have been taking this for 8 days now, unfortunately, I have not noticed any improvement in my symptoms (frequent urination, slight pain in the lower abdomen, mild burning sensation in the urethra). The doctor says I should still take it for a total of 20 days, then we can test if the pathogen is still detectable, if so, I may need to go to the hospital.
Do you think it is wise to continue taking the antibiotic? Or should I go to another doctor / hospital?
Male, 34 years old
Dear inquirer,
I would recommend continuing the antibiotic for at least another 10 days. Additionally, you can take over-the-counter anti-inflammatory medication such as Ibuprofen 400mg twice daily temporarily. If there is no improvement, I would recommend seeing a urologist again. I am not aware of the microbiology results, but if symptoms persist, a hospital antibiotic therapy may be necessary. Especially if you start to develop a fever above 38.5°C, or if you begin to experience flank pain, you should seek medical attention.
It is unusual for a young man to have a urinary tract infection. Further investigation should be done to determine the cause (anatomical reasons, sexual activity, etc.). If an infection from sexually transmitted diseases cannot be ruled out, the antibiotic treatment should be expanded with, for example, Doxycycline.
In some cases, it may take longer for the symptoms of a urinary tract infection to completely disappear.
Get well soon.
