very heavy bleeding

I have been experiencing long periods frequently for the past few months. My gynecologist prescribed me Chlormadinon 2 mg, half a tablet for 12 days. I stopped taking it after 12 days, but on the 14th day I had the heaviest bleeding I have ever experienced, and I am afraid of bleeding too much. Can I start taking Chlormadinon again (I tolerated it well) to stop the bleeding? I will see my gynecologist next week. Can I start taking Chlormadinon again today?

Dear Manuela,

I would advise you not to do this, but to go to a gynecologist as soon as possible. Your gynecologist prescribed the medication in a very low dosage, yet you are reacting very strongly. Since the bleeding occurred 2 days after discontinuation, it is not a normal menstrual bleeding but rather breakthrough bleeding, which is described as a side effect of the medication and can occur. However, it seems to be so severe that you are already worried about bleeding too much.

I would also advise you to consider a dilation and curettage with hysteroscopy, which is an inspection of the uterus with an endoscope. This can be done by almost all gynecological departments of hospitals. This can help rule out other causes (such as polyps, anomalies) for the heavy bleeding in your case.

I am sorry that I cannot give you a simple advice, but in your case it seems best to investigate the cause of the bleeding. The attempt with the Chlormadion must be considered unsuccessful. Prolonged use beyond 12-14 days is also not recommended by the manufacturer. Whether you should make a second attempt, for example in combination with an estrogen, is something you should discuss with your gynecologist. Perhaps you can call him on Monday and discuss the possibility of a dilation and curettage if the bleeding persists.

With best regards,
Dr. R. Berg
