Spermatoceles and sudden changes in the testicles

Hello, I need a second opinion. About 2 years ago, a spermatocele was diagnosed in the right epididymis. About a year ago, I noticed that the right testicle now hangs significantly lower than the left (a significant change from before). Two months ago, another spermatocele was diagnosed on the right side. I also now experience pain in the right scrotum. When asked about the further changes in the testicles, my current urologist informed me that these are now issues I have to live with. I have since noticed that the right testicle no longer moves with different abdominal muscle contractions, but the left one does. Therefore, I clearly associate the problems with each other. I am now seriously considering having the spermatoceles removed. Is it likely that the described problems will be resolved with this? Family planning is already completed, so this is not a hindrance in my opinion.

Hello, your question can at least be answered in part.

Puncturing a spermatocele is possible, but unfortunately it almost certainly leads to a recurrence.

Surgery can safely eliminate the spermatocele, but is only necessary if the finding is locally bothersome or is causing symptoms as safely as possible.

A small spermatocele is unlikely to cause a lower testicle or immobility. Do you know the size of the spermatocele?
