such "start-up pains" or "start-up problems" are typical for early signs of wear and tear. That's why the arthroscopy was done, and presumably they tried to smooth out the cartilage and remove small cartilage particles that could cause impingement. Unfortunately, these procedures often do not bring the relief one hoped for. It is often recommended to buy "rebuilding injections", but this also often does not have the desired effect.
A pragmatic approach is to factor in these start-up problems, still move a lot, and if possible engage in a sport where the knee is moved without much strain, such as swimming or even better, cycling. Cycling trains the thigh muscles, which are the only truly effective help for the knee. Many patients manage well for years and decades with this approach, without needing surgery.
