Clostridium difficile infection

I am a wheelchair user and, after taking antibiotics for a urinary tract infection (Cefuroxime), I developed diarrhea two weeks later, which lasted continuously for 21 days. In the meantime, a lab test showed that I have Clostridium difficile, with positive toxins. On day 17, I was prescribed Metronidazole, but had to stop taking it four days later due to extreme intolerance. The medication did help with the diarrhea though: I was actually diarrhea-free with solid stool from day 21 to 23! I then started taking Vancomycin, which I have been taking for 4 days now (125mg 4 times a day). Today is day 25 of the illness, and the diarrhea has returned! The diarrhea has returned WHILE taking Vancomycin. Additionally, I have had abdominal pain and intestinal noises/bloating since today. Alongside the antibiotic treatment, I have also been taking one Perenterol capsule twice daily for the past 8 days.

What happens next? I tolerate Vancomycin well and will continue taking it for another 5 days until the course is completed (package is empty). But if I am experiencing diarrhea again now - what does that mean? It doesn't seem to be working? Which doctor should I consult, who are the experts in the Rhein-Neckar region? Should I go directly to the university hospital? Would the next step be the extremely expensive Dificlir or even a stool transplant? I am privately insured and was already shocked at the price of Vancomycin! But the insurance will cover everything, if necessary, right? Apart from the slimy and very watery diarrhea with bloating and food intolerance, I have not experienced any severe symptoms, such as high fever or anything like that.

Good evening, I am happy to help you further.

Your insurance will most likely cover the costs, as you urgently need these antibiotics. Please see a gastroenterologist (it doesn't have to be at the university hospital, it can also be a private practice colleague). There are different treatment regimens that can be applied after a resistance test of the Clostridia has been conducted.

Perenterol is available in a regular and a strong version and can be taken three times a day.

Please drink plenty of fluids to replace the liquid you lose during diarrhea.

Do you have any questions? I am happy to help you further.

All the best and a speedy recovery!

Best regards,
