Tingling and numbness in toes after standing up on one side.

Hello,
I am a 34 year old male with the following pre-existing conditions:
- Overweight
- Lower back pain orthopedically cleared (acupuncture, back exercises)
- Fasciculations for 1 year all over the body, mainly in the calves (Neurologically no findings)
I have started experiencing a sensation of heat in my right foot, mainly in the toes, and now my right foot tingles daily after waking up. Additionally, my toes feel numb. This sensation goes away after movement and occurs occasionally throughout the day. Occasionally, my foot hurts slightly.
What could this be?
Good day,
Such symptoms arise in the lower lumbar spine, so they are related to the back. Either the nerves there are irritated by muscle tension or by a herniated disc. The fact that it improves after movement suggests that it is not (yet) a surgical emergency.
Nevertheless, a further neurological assessment should be carried out promptly and treated with anti-inflammatory medication (e.g. cortisone injections), even though you have already been examined. Obviously, there has been a deterioration that can also spread significantly.
Your gymnastics should be carried out cautiously until further assessment and at the same time, a calorie-reduced diet should be initiated to counteract the overload on the back.
I wish you all the best!
Sincerely, Dr. Gehring
