urinary incontinence

Hello!
I am a 34-year-old male and have had problems with my bladder for as long as I can remember. I have to go to the bathroom about 10 times a day even though I drink very little during the day. I know you should drink 2-3 liters a day. It's very bad when I drink coffee or alcohol, then I have to go to the restroom every 5 minutes. It is very noticeable when I go out for drinks with friends. During the same time and same amount of alcohol, I have to go to the restroom 8-10 times more often than my friends. As soon as I come back from the restroom, I have to go again. I can't hold it in anymore and it always has to go very quickly. It has happened several times that I have had accidents in my pants. I don't know what to do anymore, I have been to a urologist who performed various tests on me, but unfortunately couldn't find anything and said that it is normal. However, it is not normal to have to go to the bathroom so often and not have it under control and always have to look for the next restroom. I hope you can help me get a handle on this "big problem" for me.
Thank you very much in advance.
Kind regards
Dear Questioner,
It is really not normal that you have to go to the toilet so often, since you have already been to the urologist, I think it is now time to also consider other, metabolic/endocrinological causes.
I would recommend you to see an internist and especially to test the pituitary gland anterior lobe hormone ADH (Vasopressin). This hormone leads to too many "outflow channels" being built into the kidney when it is released in excess, causing you to need to go to the toilet very often. However, you would also have a strong thirst throughout the day. Is that the case for you?
Other possible causes can be in the kidney or other urinary tract. In this case, you should speak with a nephrologist, a specialist for the kidney.
Furthermore, it could actually still be a urological problem, possibly requiring 1. an examination of your prostate 2. an X-ray (excretory urogram) of your bladder and the course of the urinary system.
What is important now is that you may first see an internist, who can also test for other metabolic disorders. They should then initiate and coordinate referrals to a specialist, and can also help you interpret the findings.
You are still very young, so in my opinion it is very important that this matter be thoroughly investigated! Otherwise, go to a center (e.g. university hospital) if you cannot find help elsewhere.
If you have any further questions, please feel free to ask.
