painful urination

For about 4 weeks, I have been experiencing pain while urinating, but not constantly. Additionally, I have intermittent symptoms such as a sensation of heat and inflammation in the upper part of the penis, inflammation of the testicles which then take on a very dark red color. Sometimes, after urinating, a slightly viscous clear secretion flows out in small amounts or is flushed out. Pain in the lower abdomen occurs from time to time and is then extremely strong. There are no problems during sexual intercourse, but both the consistency of the sperm and the color are slightly altered. Normal blood and urine tests have so far been inconclusive. Tests for fungi have not been conducted. This situation worries me and I would like to know if a more thorough examination for sexually transmitted diseases would be necessary. This has also not been done yet. Would this be visible in a normal blood test? Furthermore, there are still problems with a burning sensation in the mouth and nasal mucosa, as well as a coated tongue for about 3 years, along with regularly occurring cold symptoms.

As a gynecologist, I am often confronted with the problem of sexually transmitted diseases, so I can advise you as follows:

Since the symptoms have been present for 4 weeks, a swab from the urethra should be taken and tested for bacteria and fungi.
If the urine was only tested with a stick at the doctor's office, it should also be sent in for further analysis.
Additionally, the semen should be examined. Since none of these tests have been carried out yet, you should specifically ask your urologist about them.
Possible causes for these symptoms could be fungi, chlamydia, ureaplasma, or gonorrhea. In all cases, partner treatment is recommended, so your partner should also see a gynecologist, even if she has no symptoms - the infection can be present asymptomatically in both women and men.

The dry nasal and oral mucosa, as well as frequent cold symptoms, should be assessed by your primary care physician. However, these are unrelated to the current issue.
