Pain in the urogenital area

44 years old, male
On March 22nd, I consulted my primary care physician with mild but constant urge to urinate. A urine test revealed slightly elevated leukocytes and a low level of bacteria.
Prescription: Antibiotics for 1 week and to return if symptoms persist.
On April 2nd, after symptoms seemed to have disappeared but then worsened, I went back to my doctor. The urine test showed the same results. Prescription: Nitrofurantoin 100mg twice a day for 14 days. Symptoms improved while taking the antibiotics. After the 14 days, symptoms returned.
On April 23rd, I went back to my doctor and the urine test was normal (likely due to continued antibiotic use). I was referred to a urologist.
On April 26th, I saw the urologist and the urine test was normal despite stopping the antibiotics 3 days prior. However, symptoms persisted.
Blood test for PSA and microbiological examination of urine were taken.
Despite acute symptoms, especially when sitting (in the pelvic floor area), there was no further prescription or treatment given. A new appointment was scheduled for 2 1/2 weeks later. Symptoms had since spread to the ureters. Upon further inquiry with the urologist, I was told to take Ibuprofen for the symptoms and the appointment was moved up to May 7th. Test results were requested over the phone and I was told they were "OK" but further details would be provided at the next appointment.
Meanwhile, I have experienced slight burning in both kidneys and pain along the ureters, as well as in the testicles and sensitive pelvic floor.
As symptoms are now affecting the kidney area, I am concerned whether I can wait until Monday. Is the treatment path I have described typical in such cases (treating acute symptoms with pain medication until the next appointment)?

Good evening,

I must agree with you that your course of treatment did not go optimally. You were given various antibiotics, the symptoms improved, but worsened again after stopping. Now you are showing signs of an ascending infection from the ureter to the kidneys, possibly involving the prostate. Pain medication can alleviate this but not treat the root cause. Monday is not far off, but only treating with pain relief until the next appointment is not optimal. If your symptoms continue to worsen, please seek immediate help from a hospital with a urology department or an emergency practice. The cause of your symptoms needs to be investigated.

Wishing you a speedy recovery and all the best,

Dr. M. Schröter
