Muscle/nerve twitching in the entire body

Dear Sir or Madam,

I am seeking your advice if you are a neurologist and have experience with the following. For about 4 months now, I have been experiencing regular twitching in various parts of my body.

The twitching usually occurs 3-15 times in quick succession in one part of the body. The next twitch in a different body part follows at intervals of seconds, minutes, or even hours. For example, in the biceps, then in the back or leg. However, the most common affected areas are both calves.

The twitching is not only noticeable but also visible outwardly and usually occurs during periods of rest (while standing, sitting, or lying down - often at night).

In my situation:
- I have been suffering from increased anxiety about serious illnesses for about a year
- Within this year, various examinations have already been carried out: gastroscopy, colonoscopy, liver biopsy, ultrasound and MRI of the upper abdomen, chest X-ray, various blood tests
- In addition, neurological examinations: MRI of the head, lumbar puncture, VEP (eyes), electrophysiological examinations (however, not on all twitching body parts)
- The examinations revealed the following:
1) slightly elevated liver values (recently back to normal)
2) mild, chronic gastritis
3) a small scar on the MRI of the head, but according to the doctor, without pathological significance

Now I am very worried that I have a nerve/muscle disease causing the twitching. I am thinking of ALS. Are you familiar with the symptoms? If so, what do they look like exactly? Is muscle twitching a main symptom?

My treating doctor sees the psyche as the main focus, can it cause such strong and regular twitching? I have also tried magnesium before, but without success.

I am 28 years old, male, 1.87m tall, and 84kg.

Your case seems rather complicated and not clear.

A precise diagnosis through online consultation does not seem sufficient in your case.

Anamnesis can only be conducted incompletely at this point. Likewise, due to the complexity, an excerpt from your medical history is not conclusive.

A complete neurological examination, possibly a nerve conduction velocity test, an internal medicine examination, as well as a laboratory examination are, in my opinion, essential.
