Hello, I have been experiencing pain in a tooth and the surrounding bone for weeks. Four weeks ago, I was diagnosed with a purulent sinus infection, which was treated with Penicillin. The dentist performed a root canal on the affected tooth, but said that the pain was not coming from the tooth and that the roots were protruding into the sinuses. However, the ENT doctor told me that after taking the antibiotics, the pus had disappeared. Then the symptoms got so bad that I had to take around 3 600mg Ibuprofen per day over the weekend and had a CT scan of the sinuses on Tuesday. Now the ENT doctor is on vacation and the pain is getting worse again. The report states:

No evidence of pathological findings in the right maxillary sinus, ethmoid labyrinth, frontal sinus, and sphenoid sinus. In the alveolar recess of the left maxillary sinus, there are convexly bordered structures with up to 1.3 cm in width showing pathological findings. The bony boundaries of the paranasal sinuses are intact. In the nasal cavity, there is a slight deviation of the nasal septum to the left with moderate hyperplasia of the inferior nasal concha on both sides.

Assessment: 1.3 cm mucosal retention cystic changes in the alveolar recess of the left maxillary sinus. No signs of secretion blockage.

What does the report say? The throbbing has started again :( and the pain is increasing. I would appreciate a response.

Hello,

It is very likely that the problem is due to irritation of the trigeminal nerve. This condition, known as trigeminal neuralgia, may originate from a purulent sinusitis infection.
The findings are actually normal and I no longer suspect an active process in the sinuses as the cause.

I would recommend a treatment with carbamazepine by a neurologist who can confirm this suspicion.
