Independent muscle contraction in the morning

Good day,
I have the following problem and so far I have not found anyone who can explain it correctly. Every morning after waking up, my muscles contract violently without any action on my part, with every movement. Specifically, when I move my foot or leg, my abdomen, hips, and thighs contract quite strongly, it can also extend to the chest, back, shoulders, etc. after a few movements it returns to normal. The condition is very difficult to control, sometimes I manage to reduce the contraction through targeted relaxation, but most of the time I just have to wait it out. Interestingly, it only happens in the morning, if I nap for an hour during the day, nothing happens. I have observed this condition since a hip and knee replacement surgery 3 years ago, sometimes more, sometimes less. I suspect it comes from the back, as there is almost nothing left between the vertebrae in the lumbar spine, and I also feel pain in my back.
I know that a remote diagnosis is quite vague, but maybe someone with experience with such complaints or who has treated a patient with similar symptoms before will be able to help. I would like to know where to start. Although I am in very good hands at the University Hospital in Basel, I would like to avoid getting caught up in the cycle of tests (EMG, CT, MRI, etc.) without already having a vague idea of what or where to look.
Thank you for any competent response.

Dr. med. Frauke Gehring

Good evening,

An issue in the area of the lumbar spine is to be expected, as your symptoms only occur after a long period of lying down. EMG and MRI are therefore recommended examinations. I suspect that muscle tension caused by lying down is exacerbating the cramps.

However, that alone may not be the only cause. I have seen such cramps due to calcium deficiency. It would also be advisable to check magnesium and potassium levels, especially if you are taking water-excreting tablets.

All the best!

Sincerely, Dr. Höllering
