Recurring pain in the lower abdomen including hardening etc Dizziness Pressure on the forehead PLEASE HELP

Hello!
I, male, 36 years old, weight 130 kg at 1.86m height, smoker have been having the following problems for about 1.5 years now. It all started with occasional discomfort around the navel. Shortly after, I also started experiencing back pain (just before the buttocks). These pains became stronger over time. My navel also became slightly inflamed (yellowish discharge). Additionally, I experienced constant diarrhea followed by constipation. I also had constant bloating. I visited my doctor and underwent a thorough check-up. I was given a cream for my navel. The ultrasound and blood tests showed no abnormalities. When my symptoms worsened after a few weeks (especially bloating and diarrhea), I went back to the doctor. He asked about stress, tight pants, etc.! I could rule out all of those factors. I then insisted on seeing a gastroenterologist. I underwent breath tests for lactose, fructose, and sorbitol. All results were negative. The stool and blood tests also showed no abnormalities. As my back pain continued to worsen, I was referred to an orthopedist. He performed an MRI scan which revealed some wear and tear and my poor posture, but nothing significant. He recommended physical therapy and explained that my other symptoms were not related to my back. My back slowly improved, but I still experience pain in certain situations. In addition to the remaining problems (irregular bowel movements, bloating), I started experiencing severe abdominal pain (around the navel or just above it) more frequently in the past year. In situations where I had to stand for a long time or was under pressure, a fist-sized lump would form above my navel, everything would become tense, and the pain would intensify. It felt as if someone was stabbing me in the stomach with a knife (hard to explain). It became unbearable at times, and I had to lie down immediately. The pain would gradually subside when lying down/sitting and with some rest, and within an hour, my abdomen would soften again (no more lump, etc.). The pain would also diminish. I noticed that my abdomen would pulsate visibly (despite the "fat") during intense physical activity. The longer I exerted myself, the more intense the pain would become, and it would last longer, and my abdomen would become harder. I went back to my doctor with all my previous test results. Another ultrasound and comprehensive blood test showed no abnormalities. The only thing that was noted was that my blood pressure was consistently high (long-term monitoring). I was referred to an eye doctor and advised to lose weight. Since then (December 2016), I have been experiencing increasing dizziness, constant pressure on my forehead and eyes, and tight facial skin (including flushed cheeks). I also noticed slightly bloody mucus when blowing my nose. I find it difficult to have extended conversations as the pressure increases and my concentration decreases. Sometimes I even feel like I am slurring my words. I was then sent to an ear, nose, and throat specialist. Polyps were found in my nose. A skin prick test was performed, and I tested positive for dust mites, grasses, and rye. I was prescribed a nasal spray and tablets. Now I am supposed to undergo immunotherapy. However, my symptoms (pressure, dizziness, feeling detached from reality, etc.) have worsened since then. I feel okay in the mornings but it gets worse as the day progresses. It's really bad! I feel most comfortable lying down on a sofa or in bed. I can hardly meet my daily responsibilities. I now measure my blood pressure daily, and it's usually around 150/100/65. I am at a loss as to what to do next. I have visited my doctor, gastroenterologist, eye doctor, and ENT specialist at least 5-6 times.
The constant dizziness (daily for almost 2 months now) and the recurring abdominal pain and lump around the navel in the described situations (at least 2 times a week) are my biggest concerns. I often feel like something is about to burst in my abdomen.
What could be causing this, what else can I do, and how should I proceed???
Thank you!

Good day,

Please consult a surgeon for your recurring symptoms, as they suggest a hernia. The shooting pains may indicate a strangulation.
More important than weight loss is to quit smoking. It is possible that this (along with high blood pressure and possibly elevated blood fats) has already caused circulation disorders in the brain.
Therefore, please make sure to move around a lot, if the pain allows it!
At a certain point, medication may not be enough, so a change in lifestyle is essential.
All the best for that!

Sincerely, Dr. Höllering
