High blood pressure and pulse cause unknown

Hello,

On 8.01. in the evening, I measured my blood pressure, it was 200/110 with a pulse of 110. Within a few minutes, it accelerated to 150. I suspect that panic also contributed to this blood pressure/pulse. Apart from nervousness, no pain or other symptoms.
We called an ambulance. I was given tablets. The blood pressure dropped to around 160/100 and the pulse went below 120.

I then went to the hospital.
According to the documents, I was given "Tavor Expidet".

Several tests were conducted with the following results:
Good blood values including the thyroid.
Very good ultrasound on the heart,
Regular heart rhythm.
Suspicion: Psychosomatic.

I was advised to see my general practitioner. I initially did not follow this advice (big mistake) because I started feeling better in the following days.

When I measured a value of 190/100 with a pulse of 100 on Thursday (19.01) after 3 hours of sleep, I went to the general practitioner on Friday. I was even let in earlier. This calmed me down.

The general practitioner prescribed me Bisoprolol ratiopharm 1.25 mg (once daily) as well as Atosil drops in case of emergency. I have a new appointment next Friday.

I am puzzling over the reasons for the high blood pressure...
Differences from last year?
Very little exercise and moving about for about a month.
I now leave the house less often because it's so cold and I'm waiting for a retraining program.
This year, I started feeling very restless, waking up frequently. I usually only fall asleep around 4 o'clock. This started a few days after I started taking Folsan 5mg and Vigantoletten IE1000, one each in the morning. Honestly, I was happy to be so awake. After all, I was as productive as I had been in years!

This Friday at the doctor's appointment, the blood pressure/pulse normalized, regular heart rhythm.
However, after the doctor's appointment, it rose again very strongly. I noticed a connection: I took Vigantoletten/Folsan/Lamotrigine (for epilepsy for 7 years) as usual every morning, laid down in bed.
After 10 minutes, the pulse was back at 120 and the blood pressure values at 160/100.

I picked up the Bisoprolol from the pharmacy, and it was a bit better again. Of course, I stopped taking Folsan and Vigantoletten.

Yesterday, at 6 o'clock, I had around 150/90 with a pulse of 80. At 7 o'clock, I almost had my normal values again (130/82), pulse 60. The lowest values since I started measuring it. So I did not take the Bisoprolol at first. However, after breakfast, the pulse was again over 80. I took the medication.

This morning, upon waking up, 150/82 with a pulse of 78. I will take the medication later. Definitely much calmer than on Friday.

I assumed that everything would normalize after stopping Folsan and Vigantoletten, but it did not. :-(

Even though my weight fluctuates between 85 and 90 kg at 179 cm, I have never had such problems before!
I usually have ideal values at 120/80 and a pulse of 60... I ride thousands of kilometers on my bike each year, sometimes over 300 a day/1000 a week. My diet has definitely been unhealthy in recent years. You can "afford" more chips, cocoa, and cola when you have so much exercise...

This is very unsettling for me. :-( Please advise.
My general practitioner said I should go to the hospital if it gets worse again...

What other possible causes are there? Kidneys?

...I haven't even had a cold in the past few weeks.

Dr. med. Frauke Gehring

Good morning,

The kidneys cannot be the cause, because this would have been seen in the blood in the hospital. I would initially assume a psychosomatic cause. As a precaution, cortisol and vanillylmandelic acid should be tested in a 24-hour urine sample. Your primary care physician can do this. There are rare tumors that can cause such blood pressure crises.

Typical for psychologically induced increased heart rate and pressure is the absence of symptoms under medical care. Perhaps the change in your career is causing insecurity and unconscious panic. I would explore this further.

I would recommend taking the beta blocker permanently, and having your blood levels of vitamin D and folic acid tested, only taking them if there is a deficiency. However, I do not see a connection between taking the medication and your symptoms.

All the best!

Sincerely, Dr. Höllering
