Dear Sir or Madam,
I have received a certificate from my private health insurance company regarding preventive expenses. What do I need to enter in the Annex for preventive expenses (line number 31)? The amount to be paid in 2010 or the separately listed healthcare expenses? My daughter is also privately insured under my policy. Do these expenses belong in the Annex for children (line number 31)?
Note: I am divorced and responsible for supporting my daughter.

Thank you for your inquiry, which I would like to answer based on your information and in the context of your contribution in an initial consultation as follows:
You must enter the amounts certified by the private health insurance in lines 31 (health insurance) and line 32 (long-term care insurance) and attach a copy of this certification to the tax office.
Since the child is entitled to child benefit (I assume), the entries in the Child Annex should be made in lines 31-38 and not in the Pension Contribution Annex.
