Hello, if an MRI urography including MIP of the kidneys, urinary tract, and bladder has been done, is an additional cystoscopy necessary for microhematuria? Can the urethra also be seen in the MRI? The MRI was completely normal. I understand that an additional cystoscopy would provide an extra level of certainty, but what is the likelihood that something would not be seen in the MRI? If nothing is found in an MRI for microhematuria, then indeed cystoscopy is the next sensible step to find the source of the bleeding.
Microhematuria means that very little blood is being lost. So, you are essentially looking for the source of tiny droplets of blood. These cannot be seen in an MRI, but there is definitely a chance, during a cystoscopy, to find those areas in the bladder or urethra whose surface is altered in such a way that small bleeding can lead to microhematuria.
Since it is generally important to clarify microhematuria, I consider the suggestion of my colleagues to be good and correct.
