Ulcerative colitis and pre-exposure prophylaxis
Hello!
Does taking PrEP (Truvada or generic) make sense for a male Ulcerative Colitis patient? Even in a perceived remission, there may be a slight inflammation present, especially in the rectum where Colitis usually begins. The intestinal barrier/mucous membrane/flora is damaged/disturbed. Will PrEP still work fully in this case? How does it work in clinical remission compared to during an acute flare-up? In my last colonoscopy, I was diagnosed with Pancolitis. I am now facing a treatment change. Mesalazine and Humira/Imraldi (switching from Humira to the biosimilar seemed to bring about at least a perceived remission from around the end of April to mid-November) are no longer effective. Now Azathioprine/Mercaptopurine, Entyvio, Xeljanz, possibly Stelara are being considered. Acute Prednisolone was recently slow to act and had significant side effects, so it is best to avoid it altogether. Which medication would you recommend the most, so that PrEP also works and ideally has no side effects? I have been taking PrEP since August with about a 4-week break, accompanied by check-ups. My gastroenterologist is unaware of this. Currently, my Calprotectin level is at 1300. The question mainly pertains to passive anal intercourse without a condom, which I currently do not want (please clarify this theoretical possibility as well), but perhaps in the case of only a slight inflammation..., as well as generally on all other possible practices without anal penetration, or the effect of PrEP on UC or UC medication(s). I take PrEP with Finasteride 1mg (hair loss) in the morning and usually eat much later at very irregular times.
Thank you in advance.
