Side effects of gallstones / liver hemangioma?

Dear internist,

I am reaching out to you in the hope that you can find the cause of my symptoms or provide me with ways to help narrow down the causes together with my other doctors.

My problems started 3.5 months ago:
Chills, dizziness, trembling, pressure under the right rib cage and/or on the right side under the ribs, yellow stool, dark urine, a feeling as if adrenaline is constantly being released into the pit of the stomach (similar to the feeling when startled – but in this case the sensation subsides), I could only sleep with an elevated upper body at night, lying on my back caused pressure on the chest and a sensation of the heart beating irregularly or nausea when lying on my stomach. Additionally, my bowels are constantly rumbling and I often have a slightly elevated temperature in the evenings (37.5°C), usually accompanied by fatigue and a hot head. I also feel like my eyes often focus on nothingness (daydreaming) – even though I fight against it. In the beginning, I also had the sensation of inhaling menthol: my lungs felt ethereal.

The symptoms have since improved: I can eat again in the evenings and sleep on my back with only a slightly elevated upper body. The following symptoms remain:
- Pressure / slight pain on the right side under the ribs,
- feeling as if I am on drugs (dizziness + lightheadedness),
- belching and rumbling in the bowels,
- dark urine,
- fatigue and hot head (sometimes with a body temperature of 37.5°C),
- sometimes the adrenaline sensations.

The following tests were conducted and revealed:
Upper abdominal CT with contrast agent: a 6cm liver hemangioma
Upper abdominal ultrasound: gallstones or gallbladder polyps with a maximum diameter of 9mm
Heart ultrasound: a slightly leaky heart valve
Exercise ECG: no major abnormalities
Gastroscopy: no abnormalities, no signs of reflux
Blood tests: no abnormalities, no signs of inflammation, liver values and bilirubin within normal range.

I am 39 years old, male, 1.76m tall, slim, and weigh 65kg.

The medical examinations suggested that the pressure on the right side could be caused by gallstones and that gallbladder removal could help. However, the causes of the other symptoms remain unclear.

Do you see any connections?
Are there any meaningful additional tests to uncover the cause of my peculiar condition?
