Severe redness around the anus with nighttime itching and a wound on the scrotum.

I have had a strong redness around the anus with itching at night for over a year. Additionally, from time to time, there is a wound on the lower scrotum similar to a cut (at the base near the anus). All creams and ointments have been unsuccessful. I could not identify any dietary connections either. Sometimes at night, the itching is strong and I wake up from scratching vigorously. What could this be and which doctor or clinic can treat this? Pictures are attached.

Dear questioner, unfortunately you did not mention which ointments you have used. This looks like a fungus or eczema. You could try multilind or Hametum ointment. If this has been going on for a long time, please consult a dermatologist or urologist/proctologist. Regards, welsch
