I got a larger hematoma on my foreskin during intense intercourse.
I got some Heparin cream from the pharmacy and applied it.
Now my question: Do I need to be careful that the cream does not come into contact with the glans?

No, you don't have to be careful if it's an ointment and not a gel. However, you shouldn't expect too much from this treatment. Carefully cooling should help, for example with a wet washcloth. The rest will be taken care of by time.
