What are the consequences if I commit a hit-and-run?

I hope you can help me with my concern. My name is Uwe Zimmermann and I recently caused an accident in which I committed a hit-and-run. I panicked and left the scene without leaving my personal information. Now I am very worried about the possible consequences of this act.
