Breast inflammation with breast implants

Hello!
I have been wearing UBM breast implants for 8 months, which have healed without complications and have never caused any problems. Now, I have been experiencing pain (pressure pain) in my right breast below the nipple for about 8 days. No fever. The pain is not in the whole breast, but only in the area directly below the nipple. After about 3 days, I started experiencing pain in the armpit as well. I went to my gynecologist who diagnosed a breast infection. I was prescribed antibiotics "Cefaclor 500" for 5 days, twice a day, and "Ibu 400" three times a day. I also did some quark compresses. Unfortunately, after 4 days of antibiotics, there has been no improvement, only the pain in the armpit has gone away. I unfortunately get the impression that my gynecologist is not very motivated to provide follow-up care due to the breast augmentation surgery I had in the Czech Republic. (His comment: "Get the number of the Czech doctor ready, the implants may need to be removed!")
Now, my question is: Why is the antibiotic not working? Should I extend the treatment or switch medications? In what case would the implants actually need to be removed?

Good morning,

I would first change the medication if there is no improvement. If you do not have a penicillin allergy, I recommend AmoxiClav 875/125 mg 2x1/day. Even better would be (if available again) Staphylex 3x2 tablets/day (3g/day) for each 10 days. The implants must be removed if ultrasounds show abscesses (pus). Of course, also if the inflammation is not significantly better after 14 days, if the inflammation values in the blood (white blood cells, CRP) are high, or if you develop a fever and the inflammation spreads. You don't need to go to the Czech Republic for the possible implant removal, it can also be done in Germany.
