Trouble with hemorrhoids?

Dear expert,

I have had a rather diffuse problem for about 3 months that I can no longer properly classify. In short, it started with problems with bowel movements. Despite having a normal soft consistency, the need to use the toilet was already indicated 2-3 hours beforehand by a pressure in the anal area, without any possibility of evacuation. When it did happen, it only occurred with medium to strong pressure. This was followed by a complete blockage for about 2 weeks, which I managed to resolve with Lacutlose. What remains is a daily pressure in the anal area, which sometimes extends towards the coccyx and in the other direction towards the front below the testicular area.

An examination by my general practitioner revealed enlarged hemorrhoids as the most likely cause. The subsequent treatment with a cream (Postericort) only provided temporary relief. Blood residues (pale) are only sometimes visible on the paper.

For a more detailed clarification, I will undergo a colonoscopy in 3 months. However, what completely puzzles me is that the difficulties vary daily. Some days I have no problems at all, while on others I constantly feel the need to use the toilet, even though it is not necessary. On other days, the pressure is only directly on the coccyx. I have also noticed that the cause seems to have an influence on erections. They are actually (attention) greatly improved, similar to a prostate massage. The symptoms are almost always gone in the morning, but they appear throughout the day. My suspicion is that it may be due to prolonged sitting (office work).

My question now is: Could it be possible that hemorrhoids may vary in size daily due to different blood flows, irritations, etc., or that they may be inflamed, or that the location or size of the swelling may change, or that they may change throughout the day due to movements? Why are the problems sometimes greater and sometimes lesser, without a permanent form of healing taking place?

Hemorrhoids can actually cause varying degrees of discomfort depending on the filling of the rectum and other stress factors. Unfortunately, you did not mention your age. In young people, one must consider inflammatory conditions (proctitis), while in older individuals, one must also consider tumors.

The foremost anal area that is affected in your case can be better evaluated through a rectoscopy rather than a colonoscopy. Therefore, I would recommend seeing a surgeon for the former, and an appointment for this is usually available at short notice. Three months is clearly too long.
